Our services are designed to foster appropriate use of the primary health facilities in promoting health, preventing diseases and addressing medical emergencies.

Services

DENTAL CONSULTATION AND TREATMENT FOR MapúaMCM EMPLOYEES AND STUDENTS

(Dental Procedures that includes Oral Prophylaxis, Simple tooth Extraction and Temporary Filing.)
  1. Fills out Patient Information Record (PIR) and Secure for an appointment
  2. Dentist performs history taking, examines the oral cavity and records dental findings.
  3. Prescribed treatment plan and medication will be given.
  1. Fills out Dental Appointment form
  2. Submits Dental Appointment Form to Department head for approval.
  3. Schedules dental treatment based On college dentist’s schedule.
  1. Fills out Parental Consent form for Dental treatment.
  2. Gives Parental Consent form for Dental treatment to parent/guardian for approval.
  3. Schedules dental treatment based On dentist’sschedule.

MEDICAL EXAMINATION

  1. Secure a Referral slip for Laboratory Examination and Physical Exam.
  2. CHSW staff will Sign Referral Form slip and advises student on the process of Medical Examination.
  3. Duly Authorized health service provider does the following
    1. CBC
    2. Urinalysis
    3. Chest X-ray
    4. Physical Examination
  4. Medical Certificate and Laboratory Examination results will be forwarded to CHSW by the provider.
  1. CHSW Sets a schedule for Laboratory examination and Medical Examination.
  2. Human Resource Management Office Disseminates information pertaining the Schedule of the Laboratory Examination and Medical Examination.
  3. Dissemination of instructions prior to Laboratory Examinations.
  4. Duly Authorized health service provider does the Medical Examination.
  5. Medical Certificate and Laboratory Examination results will be forwarded to CHSW by the provider.

NON- EMERGENCY AND EMERGENCY MEDICAL CONSULTATIONS

  1. Logs name in the Daily Attendance Form
  2. Fills out Student/Employee Health Survey Form
  3. Medical Personnel Perfoms initial Assessment of patient complaints and Vital Sign Taking:
    1. Blood pressure
    2. Cardiac Rate/ Heart Rate
    3. Respiratory Rate
    4. Pulse Rate
    5. Temperature
  4. Physician will Perform medical Examination directed to the compliant and findings of the patient
  5. Medical Personnel Carries out Physician Medical Order/s and Render Health teachings.
  6. Records all consultation done.
  1. Medical Personnel attends to the medical emergency.
  2. Conducts quick physical assessment.
  3. Does appropriate First-aid Treatment.
  4. Pulls out patient data record and notes vitals signs prior to consultation.
  5. Physician performs medical Examination directed to the illness and injury and findings of the patient
  6. Informs Parents or Guardian about patient condition, first-aid treatment done and patient should be fetch at the CHSW.
  7. Carries out Physician Medical Order/s.
  8. Charts the following Focus, Data, Action and Response of the patient during treatment.

HOSPITAL REFERRAL AND TRANSFER DURING MEDICAL EMERGENCIES

  1. Attends to patient medical emergency case.
  2. Conducts quick physical assessment.
  3. Does appropriate First-aid Treatment.
  4. Contacts ambulance for emergency transport. ( if school vehicle is available for transport it may be used in lieu of contacting an ambulance)
  5. Medical Personnel Stays and monitors patient condition while waiting for transport.
  6. Accompanies patient while on transport.
  7. Notifies parent or guardian about patient condition, treatment given and the need to transport patient.
  8. Endorse Focus, Data, Action and Response to the Medical Personnel in the Hospital.

MANAGEMENT FOR MapúaMCM EMPLOYEES AND STUDENTS WITH COMMUNICABLE DISEASE

  1. Consults with patient suspected of the infection.
  2. Refers patient for testing.
  3. Refers patient to their specialist of choice.
  4. Patient submits medical certificate and recommendations from specialist.
  5. School Physician conducts final medical evaluation.
  6. CHSW assist patient in the treatment regimen.
  7. Submits certificate of treatment completion.

REQUISITION OF MEDICAL ASSISTANCE IN SCHOOL EVENTS

  1. Produce letter of request for medical assistance in School events 1 weeks prior to date of event:
  2. Letter must contain:
    1. Student Activities involved
    2. Date and Time
    3. Specific Venue
  3. Submits letter of request for medical assistance in school events to CHSW
    Approves letter of request.
