Application Instructions
Step 1: Accomplish the Online Form.
Note: Use your Mapúa MCM Outlook accounts to register.

Step 2: Submit a clear photo of your latest School ID or any proof of current enrollment indicating the most recent grade level and school year you enrolled in (e.g. statement of account, payment receipt, etc.) to [email protected].

Step 2: Download and fill out the Request for Credit Evaluation Form (RCEF) with the final grades and subjects taken from your previous school:
College: Download
SHS: Download
JHS: Download
Step 3: Email your filled out RCEF to [email protected] and attach a soft copy of your grades, and make sure to please change the file names of the documents you will be sending:
RCEF: Last Name_RCEF_Program Applying for (example: Cruz_RCEF_Entrepreneurship)
Copy of Grades: Last Name_Grades (example: Cruz_Grades)
Step 4: Please wait for our Admissions team to acknowledge your RCEF and coordinate with you through email.
